Aboriginal Family Support Services (AFSS) provides a range of services to the Aboriginal community throughout South Australia. Through strong leadership Aboriginal Family Support Services will influence change in policies and service delivery in all areas of capacity building within Aboriginal families and communities.
The ECN has the primary responsibility of connecting with local Elders and their families to raise awareness of their aged care entitlements.
The overarching function of the ECN is to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services both internal and external to the ACCHO.
The ECN will work closely with the Elder Care Coordinator. They will also help determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continue advocacy work with clients as required.
Who we are looking for:
- The position will only be open to Aboriginal or Torres Strait Islander applicates.
- Oversee the development and maintenance of networks with the community services sector (including Aboriginal agencies, government services and those provided by mainstream NGO and not for profit sectors).
- Be an outstanding ambassador for AFSS and how we conduct business.
- Communicate in a culturally responsive manner with Aboriginal individuals, groups, families and organisations.
- Provide support to enable client access to appointments.
- Attention to detail including completion of relevant case work documentation encompassing intake.
- Support clients to identify goals and needs to ensure they get the appropriate level of care and services, which can include referrals to other specialised serviced and support
To be successful you will need
- A South Australian Working with Children Check (WWCC)
- National Police Clearance
- Safe Environments for Children and Young People Certificate
- Valid South Australian Driver’s License
- Unrestricted eligibility to work in Australia
- Aged Care Clearance
- Current First Aid Certificate
- A qualification in health or care related field is preferred but not essential
